The game begins with journalist Miles Upshur, who arrives at Mount Massive Asylum, a psychiatric hospital in search of a scoop. To his surprise, Miles discovers that the hospital is unguarded and all entrances are closed.

Having entered the hospital through the window, the journalist sees all the horror happening in it: dismembered corpses are scattered everywhere, and crazy and mutilated patients wander along the corridors. Among all these crazy people, the protagonist meets a certain priest Martin, who promotes a cult around a local deity called Walrider, a former security guard Chris Walker, stuffed with hormones, looking like a huge mutant and pursuing Miles throughout the game in order to restore order, that is, to kill, and doctor Richard Trager - a sadist who uses patients for his delusional surgical experiments. Moving along the corridors of the hospital, the protagonist learns that it is only a cover for the Murkoff scientific institute, which serves as a base for conducting illegal experiments on people in order to obtain the greatest profit and breed a new species of people with superpowers.

Miles soon learns that Murkoff's laboratory is located under the hospital. While exploring the lab, he stumbles upon a strange dark clot, similar to fog, which begins to haunt him. Suddenly, Chris grabs the reporter and tries to kill him, but that black fog intervenes and brutally kills Chris, disappearing after that. Miles moves on and finds a scientist, Dr. Rudolf Wernicke, who talks about the Walrider project and the mysterious clot - Billy - the only test subject who survived the experiments. After that, he asks to kill Billy. To do this, it is necessary to turn off the water and electricity supply to Billy's capsule, which contains his physical body. The Walrider, having lost its owner, moves into Miles's body. Miles slowly returns to Wernicke's office, where special forces are waiting for him. The screen goes dark, shots are heard in the background, and then screams. The special forces kill Miles, and the Walrider, having broken free, deals with the special forces.
